Change of Membership Details
The Membership Information Form was adopted in 2011 and was initially aimed at those members elected prior to the introduction of compulsory completion of a membership form in 2012 so that the Association could maintain an up to date register of the members of the Essex Association.
The form may also be used:
- If you have recently changed your name or address or intend to do so in the near future
- If you have signed a Gift Aid Declaration and you stop paying tax on your income and/or capital gains tax or if you change your name/address
Please use the Membership Information Form to let your District Treasurer and the Association Treasurer know of your change of membership details.
